        I, , hereby certify that all of the statements made on this application are true and correct and are made in consideration of the extension of credit by Colorado Drywall Supply, LLC.The undersigned agrees to be bound by the Terms and Conditions of sale, which are 1% discount if paid by the 10th of each month following the month of billing, or Net 30th. If payment is not made by the 30th of the month following the billing, the amount will be considered past due and the undersigned agrees to pay a finance charge of 1½% interest, or 18% per annum, but not to exceed the maximum allowable rate. If any amount hereunder or hereafter is not paid when due, then the undersigned agrees to pay all reasonable costs of collection, including, but not limited to, reasonable attorneys’ fees.



        Personal Continuing Guarantee

        In consideration of extension of credit by Colorado Drywall Supply, LLC, LLC and other good and valuable consideration, the undersigned agrees to be bound by the Terms and Conditions of sales set forth herein and does unconditionally guarantee prompt payment when due.

        The account of Colorado Drywall Supply, LLC includes credit extended herein or hereafter, and further agrees that if this account is not paid when due, to pay all reasonable costs of collection, including, but not limited to, reasonable attorneys’ fees. This guarantee shall cover the renewal of any claims guaranteed by this instrument or extensions of time of payment thereof, and shall not be affected by any surrender or release by Colorado Drywall Supply, LLC, of any other security held by it for claims hereby guaranteed.
